****PATIENT NOTICE****

AT PRESENT, THE PRACTICE IS EXPERIENCING A CRITICAL SHORTAGE OF GP COVER.

WEEKS COMMENCING 8TH AND 15TH JULY WE WILL NOT BE ABLE TO OFFER ANY ROUTINE APPOINTMENTS.   WE WILL ONLY BE ABLE TO OFFER SAME DAY GP APPTS FOR URGENT MEDICAL ISSUES.

ROUTNE CLINICS AND NURSE PRACTITIONER APPTS WILL ALSO BE AFFECTED.

WE WILL BE ABLE TO OFFER APPTS WITH THE MH PRACTITIONER, SOCIAL WORKER PHARMACIST AND PHYSIO. 

UNDER DIRECTION FROM THE DOCTOR THE RECEPTIONIST WILL REQUIRE A SUMMARY OF YOUR SYMPTOMS TO NAVIGATE TO THE MOST AAPPORPRIATE HEALTH CARE PROFESSIONAL.

 CHANGES TO PRESCRIPTION ORDERING

FROM MONDAY 5TH AUGUST 2024 THE PRACTICE CAN NO LONGER ACCEPT REPEAT PRESCRIPTION ORDERS FROM YOUR NOMINATED PHARMACY. 

PATIENTS CAN ORDER REPEAT PRESCRIPTIONS:

  • By submitting their order in writing at reception or by posting their order into the practice.
  • Online via patient access. Patients must receive a letter from the practice to register for online patient access.
  • Via our telephone messaging service on 02871253223 (from 5th August 2024)

WE UNDERSTAND THIS CHANGE WILL REQUIRE A PERIOD OF ADJUSTMENT FOR PATIENTS AND THE PRACTICE.  WE WILL PROVIDE ADMINISTRATION SUPPORT FOR PATIENTS TO SET UP ONLINE PATIENT ACCESS.  

REPEAT PRESCRIPTIONS WILL BE READY AT THE PRACTICE 48 HRS AFTER RECEIVING YOUR ORDER AND AFTER 72HRS AT YOUR NOMINATED PHARMACY. 

THERE WILL BE NO CHANGE TO PHARMACY COLLECTION FROM THE PRACTICE. 

BANK HOLIDAYS

THE PRACTICE WILL BE CLOSED ON FRIDAY 12TH JULY AND MONDAY 26TH AUGUST 2024 FOR BANK HOLIDAY.

Processing these prescriptions can cost 20 times the price of buying identical medication at your local pharmacy or supermarket

THE PRACTICE WOULD LIKE TO ASK  PATIENT'S WHO REQUIRE MEDICATION FOR HAYFEVER AND ALLERGIES TO PURCHASE THESE AT YOUR LOCAL PHARMACY OR SUPERMARKET. THANK YOU FOR YOUR CO-OPERATION.

 

**ZERO TOLERANCE POLICY**

Please note the practice operates a zero tolerance policy. In recent months we have noticed a significant increase in abuse towards our staff. At Cityview Medical we treat all our patients with dignity and respect and we expect all our staff to be treated with respect. We will not tolerate any abusive language, threatening behaviour or aggression towards any member of staff. How we use your medical records:

 

The use and sharing of personal information forms an essential part of the provision of health and care, benefiting individual patients, often necessary for the effective functioning of health and social services and sometimes necessary in the public interest. Nevertheless your doctor has a strong legal and ethical duty to protect patient information and all information you share with your doctor is kept confidential.

 



    • This practice is committed to observing the laws on data protection and confidentiality concerning your medical record and all uses and sharing of your information.

    • We share information about you with health professionals who are involved in providing you with care and treatment. This is on a need to know basis and, normally, event by event.

    • Some of your health information (including your name, address, allergies and medications) is automatically copied to the Northern Ireland Electronic Care Record.

    • Some information about you is shared with national screening campaigns such as Flu and Diabetes eye screening.

    • Information about you in non-identifiable form is used to manage the NHS and make payments.

    • Information about you in non-identifiable form is used to check the quality of care provided by the NHS.

    • Information about you in non-identifiable form may be used for medical research. Where identifiable information may be required we will first seek you consent. 

    • We will share information when the law requires us to do, for instance when we are inspected or reporting certain illnesses or safeguarding vulnerable people.

* “Common Law Duty of Confidentiality”, common law is not written out in one document like an Act of Parliament. It is a form of law based on previous court cases decided by judges; hence, it is also referred to as 'judge-made' or case law. The law is applied by reference to those previous cases, so common law is also said to be based on precedent.

The general position is that if information is given in circumstances where it is expected that a duty of confidence applies, that information cannot normally be disclosed without the information provider's consent.

In practice, this means that all patient information, whether held on paper, computer, visually or audio recorded, or held in the memory of the professional, must not normally be disclosed without the consent of the patient. It is irrelevant how old the patient is or what the state of their mental health is; the duty still applies.

Three circumstances making disclosure of confidential information lawful are:



    • where the individual to whom the information relates has consented;

    • where disclosure is in the public interest; and

    • where there is a legal duty to do so, for example a court order.   

We use a processor, iGPR Technologies Limited (“iGPR”), to assist us with responding to report requests relating to your patient data, such as subject access requests that you submit to us (or that someone acting on your behalf submits to us) and report requests that insurers submit to us under the Access to Medical Records Act 1988 in relation to a life insurance policy that you hold or that you are applying for. iGPR manages the reporting process for us by reviewing and responding to requests in accordance with our instructions and all applicable laws, including UK data protection laws. The instructions we issue to iGPR include general instructions on responding to requests and specific instructions on issues that will require further consultation with the GP responsible for your care.
