*****ZERO TOLERANCE POLICY*****

Please note the practice operates a zero tolerance policy. In recent months we have noticed a significant increase in abuse towards our staff. At Cityview Medical we treat all our patients with dignity and respect and we expect all our staff to be treated with respect. We will not tolerate any abusive language, threatening behaviour or aggression towards any member of staff. Any violation of this policy will result in removal from the practice list.

***FLU VACCINE***

This is the time of year when those over 50 years old or with certain conditions can benefit from flu vaccination. This year more than ever it is really important that all those entitled to the flu vaccine receive it. If you have not had your flu vaccine and are eligible please contact the practice to arrange an appointment with the Nurse Practitioner

FOR REPEAT PRESCRIPTIONS PLEASE ORDER ONLINE, OR PLACE YOUR ORDER IN THE POST BOX AT RECEPTION.

ALL PRESCRIPTIONS WILL BE COLLECTED BY YOUR NOMINATED CHEMIST.

PIP APPLICATIONS

As a Practice we no longer complete requests from DLA / Attendance allowance / PIP forms OR for supporting evidence.

We strongly believe that to get an accurate assessment a face to face consultation is vital.  This needs to be carried out by the agency making the decision about your application, not by your GP.

Test Results

Note that the practice has a strict policy regarding confidentiality and data protection and we will only release test results to the person to whom they relate unless that person has given prior written permission for the release of this data.

Please Note: This is applicable to all patients aged 16 and over.

When you take your test you will be told how long it will be before the results are returned to the practice.

We would ask all patients to contact the practice for their results and to make an appointment with your doctor if you are advised to do so or if you wish to discuss your test results further.

Please Note: Administration Staff are not medically trained to comment on results.
