PIP APPLICATIONS

As a Practice we no longer complete requests from DLA / Attendance allowance / PIP forms OR for supporting evidence.

We strongly believe that to get an accurate assessment a face to face consultation is vital.  This needs to be carried out by the agency making the decision about your application, not by your GP.

Welcome to Cityview Medical

The Waterside Health Centre accommodates all the General Practitioners in the Waterside in 3 practices.  There is a spacious car park nearby and car parking in the surrounding streets.  The Centre also accommodates an equipped Treatment Room with Nurses in attendance.  We also have attached District Nurses and Health Visitors and close association with the local Social Services Office.

Cityview Medical offers family doctor services to registered patients within an 8-mile radius from the Health Centre, east of the River Foyle.

As we are involved in GP training, a seventh doctor is often available for consultation.

We hope you take your time to browse our website where you will find lots of useful information about the surgery and the services we offer.

The practice now has a new diverse multi disciplinary team.

Mr Patrick Mc Randle-Physiotherapist specialises in problems with your joints, muscles and soft tissue (back pain, neck pain, painful joints or sprains or strains). He can diagnose your condition, offer advice and exercises to manage your condition, order tests, request x-ray, refer to another service if required and direct you to community based activities.

Ms Allison McCallion Mental Health Worker specialises in mental health problems including: crisis intervention, suicide prevention, stress related problems, relationship issues, depression, anxiousness and bereavement support.

You can self refer to the mental health worker by asking the receptionist to make an appointment.  The mental health worker may sometimes be able to offer an appointment sooner than the GP.  She is able to refer to secondary care and arrange medication if necessary.

Mr Brendan Burns practice Social Worker specialises in social issues including: stress related problems, relationship problems, carer support, anxiousness, benefit advice, emotional problems, safeguarding concerns, bereavement support, housing problems and obesity. The Social Worker  may sometimes be able to offer an appointment sooner than the GP.  She is able to refer to secondary care and arrange medication if necessary.  He can also liaise with core services to help support you.

We have two practice based pharmacists- Mrs Denise Mc Mahon and Miss Laura O' Donnell.  If you want to discuss your medication in detail they will be happy to assist, please ask a receptionist for details.

Access

There is wheelchair access to all the surgeries and treatment room and designated spaces in the Health Centre car park for disabled drivers.
